For home school students, the following form is required.

To participate in athletics at EVA, all students must have the following forms completed and on file with the athletic director.

> FHSAA  EL-7 - Registration Form for Home Education Student

> FHSAA  GA4: Affidavit of Compliance for Recruiting and Participation

> FHSAA  Form EL2: Pre-Participation Physical Evaluation

For new athletes to Eagle's View Academy going into the 7th, 10th, 11th, or 12th grades, the following form is required.

 > FHSAA  EL3: Consent and Release from Liability Certficate
